Terms of Service
These Terms of Service ("Terms") govern your use of the Veritide Health platform at portal.veritidehealth.com (the "Portal"), operated by Major Weekend LLC d/b/a Veritide Health ("Veritide," "we," "us"). By creating an account or using the Portal, you agree to these Terms.
1. What Veritide is — and is not
Veritide is a technology platform that connects patients with independent, licensed healthcare clinics and helps those clinics manage scheduling, communication, and care workflows. Veritide is not a medical provider and does not practice medicine. All medical evaluations, diagnoses, prescriptions, and treatment decisions are made solely by the licensed clinic and its providers. The clinic you are matched with is responsible for your care.
2. Eligibility and accounts
- You must be at least 18 years old to use the Portal.
- You agree to provide accurate, complete information — including in medical intake forms — and to keep your account information current. Providing false health information can result in unsafe care.
- You are responsible for maintaining the confidentiality of your login credentials.
3. Text messaging (SMS) terms
- Program: If you opt in by providing your mobile number and checking the SMS consent box, Veritide will send appointment confirmations, reminders, and cancellation notices by text message.
- Frequency: Message frequency varies with your appointment activity, typically 1–4 messages per appointment.
- Cost: Message and data rates may apply according to your mobile carrier's plan.
- Opt-out: Reply STOP to any message to cancel. Reply HELP for help or contact support@veritidehealth.com.
- Privacy: Mobile numbers and opt-in data are not shared with third parties for marketing. See our Privacy Policy.
- Carriers: Carriers are not liable for delayed or undelivered messages.
4. Scheduling and communications
Appointment times shown on the Portal are set by your clinic and may be rescheduled or cancelled by the clinic. The Portal may send you service communications (email and, if you opt in, SMS) about your account and appointments. These communications are not a substitute for direct communication with your clinic in urgent situations.
5. Payments
Fees for medical services are set by and payable to your clinic under its own policies. Clinics using the Veritide platform pay Veritide a platform subscription fee; applicable terms are presented to clinics at subscription.
6. Acceptable use
- No use of the Portal that is unlawful, fraudulent, or harmful to others.
- No attempts to access other users' data or to probe, disrupt, or circumvent Portal security.
- No use of the Portal to submit information about any person other than yourself without authorization.
7. Intellectual property
The Portal, including its design, software, and content (excluding your personal information), is owned by Veritide or its licensors and protected by law. We grant you a limited, revocable, non-transferable license to use the Portal for personal, non-commercial purposes.
8. Disclaimers and limitation of liability
The Portal is provided "as is" without warranties of any kind, express or implied, to the fullest extent permitted by law. Veritide does not warrant uninterrupted or error-free operation. To the fullest extent permitted by law, Veritide's total liability arising out of or relating to the Portal is limited to the amount you paid to Veritide (if any) in the twelve months preceding the claim. Veritide is not liable for the acts or omissions of independent clinics or providers.
9. Changes and termination
We may update these Terms from time to time; material changes will be posted on this page with an updated date. We may suspend or terminate access for violations of these Terms. You may stop using the Portal at any time.
10. Governing law
These Terms are governed by the laws of the State of Florida, without regard to conflict-of-law principles. Venue for disputes lies in the state or federal courts located in Broward County, Florida.
