Veritide Health ("Veritide," "we," "us"), operated by Major Weekend LLC, provides a telehealth platform connecting patients with independent licensed clinics. This policy describes how we collect, use, and protect your information when you use portal.veritidehealth.com (the "Portal").
Information we collect
- Account information: name, email address, state of residence, and password.
- Health intake information: information you provide in medical intake forms, which is shared with the licensed clinic that accepts your request so they can evaluate and treat you.
- Contact information: your mobile phone number, if you choose to provide it.
- Usage information: basic technical data such as browser type and pages visited, used to operate and secure the Portal.
How we use your information
- To connect you with licensed clinics and support your care.
- To send service communications about your account and appointments (email, and text messages if you opt in).
- To operate, secure, and improve the Portal.
- To comply with legal obligations.
We do not sell your personal information.
Text messaging (SMS) program
- Opt-in: You receive text messages only if you provide your mobile number and check the SMS consent box on the Portal. Messages are limited to appointment confirmations, reminders, and cancellation notices.
- No third-party sharing: Mobile phone numbers and SMS opt-in consent are not shared with or sold to third parties or affiliates for marketing or any other purposes. Text messaging originator opt-in data will not be shared with any third parties.
- Message frequency: varies with your appointment activity, typically 1–4 messages per appointment.
- Message and data rates may apply.
- Opt-out: reply STOP to any message to stop receiving texts. Reply HELP for help.
How we share information
- With your clinic: your intake and appointment information is shared with the licensed clinic providing your care.
- With service providers: we use vendors (such as secure hosting, database, email, and SMS delivery providers) that process data on our behalf under confidentiality obligations.
- As required by law: when legally compelled, or to protect safety and prevent fraud.
Security and retention
We use industry-standard safeguards, including encryption in transit, access controls, and row-level database security. We retain information for as long as needed to provide services and meet legal obligations.
Your choices
- You may update your account information on the Portal.
- You may opt out of text messages at any time by replying STOP.
- You may contact us to request access to or deletion of your information, subject to legal and medical record-keeping requirements.
Children
The Portal is intended for adults 18 and older and is not directed to children.
